Home Care Qualifications and Cost

To qualify for home care services, you must be a Medicare patient who is homebound, in need of intermittent care, and under the care of a doctor. In addition, if you require professional medical assistance to remain at home, you may request our services. We can also care for you if you have come from other healthcare facilities and reside in Will and Southwest Cook counties. Medicare, Medicaid, HMOs, and various private insurance companies usually cover costs. Our staff will help research your coverage prior to beginning services.

Personalized Care Plan

Our nurses will assess your health status and provide an individualized plan that includes skilled nursing services such as wound care, catheter care, injections, infusion therapy, and medication management. In addition, they will:

• Carefully monitor your progress and report any changes in your condition to your doctor.
• Teach you or your caregiver how to manage your health care needs independently.
