Silver Cross Advanced Heart Failure Clinic Designed to Keep Patients Healthy at Home

Patients with advanced heart failure receive excellent care as inpatients at Silver Cross Hospital in New Lenox. But the development of a new outpatient program will help patients live longer, better lives and avoid repeat hospitalizations – a common occurrence with advanced heart failure patients. 

Located in Suite 450 of Pavilion A, the new Advanced Heart Failure Clinic at Silver Cross offers specialty care for patients with advancing heart failure. A team of advanced heart failure and transplant cardiologists, nurse practitioners and nurses partner with a patient’s primary care doctor and cardiologist to provide comprehensive care while offering the latest technology and approaches to treat heart failure. 

Services offered include consultation with heart failure specialists, individualized treatment plans and visits with advanced practice nurses for symptom monitoring, medication management and education. 

How it Works

Ideally, the clinic sees patients with advanced heart failure seven to 14 days after they were released from the hospital. Individual clinic appointments are offered several times a week. The first was in late January of this year.  

The main focus is educating patients, optimizing life-prolonging treatments and partnering with their physicians to help patients live longer, better lives. For some, who may have more advanced disease, experts at the Advanced Heart Failure Clinic can help to determine if these particular patients would benefit from a cardiac transplant or cardiac-assist device.

How often a patient visits the clinic depends on their situation. The clinic is prepared to support the patient over a critical period of their illness to optimize their care and prevent readmission to the hospital.
