Pre-application for Medical Staff Appointment

Dear Doctor:

Thank you for your interest in Silver Cross Hospital and our community. Our Board of Trustees, as recommended by the Medical Staff, has adopted requirements for application, some of which are listed below. Please know that these are baseline standards. Our Credentials Committee will conduct a full review of all completed applications prior to making a recommendation to the Board.

Minimum standards for application for Medical Staff membership are listed below:

1. Current license to practice medicine/dentistry/podiatry in the State of Illinois.
2. Current Federal Narcotics License.
3. Current malpractice insurance with a minimum of $1,000,000/$3,000,000.
4. You must have an office within the service area of Silver Cross Hospital as determined by the Board.
5. Plans to use the hospital for the care of at least 6 patients per year, and will contribute time to Medical Staff Activities. (In accordance with the Medical Staff Bylaws, a staff member will attend, admit, or be involved in the treatment of at least 6 patients per year at Silver Cross Hospital unless waived by the Chairman of your Department).
6. Attend at least 50% of your department meetings and 50% of staff meetings per fiscal year (May 1 through April 30).
7. Certificates from Specialty Board or letter indicating registration to take the examination. (You must be certified by the appropriate specialty board within 5 years of completion of training).
8. Podiatrists are required to have completed at least a 12 month podiatric surgical residency accepted by the American Board of Podiatric Surgery (ABPS) and approved by the Council on Podiatric Medical Education of the American Podiatric Medical Association. In addition, the candidate shall have completed successfully the written eligibility examination.
9. Have no unverifiable gaps in their clinical practice history for a consecutive period of two years or more.

Your completed pre-application, along with copies of licenses and certificates are to be submitted to the following address, or e-mail address, within 30 days of receipt of this document. An application will be forwarded to your office if it meets criteria.
